The earliest I can go with all members alive is Roxy Music. But there must be someone from the sixties? As mentioned Fleetwood Mac lineup before Danny Kirwan joined. Also Mayall's Bluesbreakers Beano lineup (and probably a couple more, too hard work to check)

Tony Meehan and Jet Harris (both now dead, I believe) left in 1962, and Brian Bennet and Licorice Locking replaced them (both still alive), but Locking left the following year and John Rostill, who replaced him on bass, killed himself in 1973. One other (mildly) entertaining fact about this line-up: as both Locking and Hank Marvin were christened Brian, that version of the Shads consisted of three Brians and a Bruce.

The Turtles' original drummer Don Murray died, but I'm pretty sure everyone who was in the band from 1967-1970 (when they were in their fullest flower creatively) is still alive.
